Our Cremation Specialist will assist you with any questions at the initial arrangement conference.
We will assist you in determining the number of certified death certificates you will require and can order them for you. Some information that will be needed to complete the Death Certificate includes:
Birth Date
Birth Place
Father'sName
Mother's Name, including Maiden Name
Social Security Number
Veteran's Discharge or Claim Number (DD 214)
Education Completed
Your Cremation Specialist will prepare Social Security Form SSA 721. Check with Social Security to see that the number has been retired.
If you are planning a memorial service, contact your Clergy.
Make a list of immediate family, close friends, employer and/or pertinent business colleagues. Notify each by phone.
Gather obituary information including age, place of birth, occupation, college degree(s), memberships held, military service, outstanding work, list of survivors in immediate family, and provide time and place of services. You may elect to also submit a photo of your loved one.
If Social Security checks are automatic deposit, notify the bank of the death.
Notify insurance companies.
Locate the will and notify lawyer and executor.
